Organizationally, where does LHC Group fit into the UnitedHealth Group enterprise? LHC Group is part of the Optum Health — Home & Community Care Solutions business within Optum which focuses on home and auxiliary care delivery.
When will LHC Group be fully integrated and what does that mean? Integration is a multi-step process and LHC Group and Optum leadership have been working together to identify opportunities for alignment and collaboration. Integration does not occur all at once — it is completed in phases (e.g., HR/people, clinical programs, technology) and varies based on business needs.
What is the People Team? That’s the term UnitedHealth Group uses to describe its Human Resources (HR) team.
How are integration decisions made? Integration decisions are made through a collaborative approach involving representatives from LHC Group and Optum/UHG. These representatives participate in various workstreams covering all business areas, such as operations, payroll, finance, people, accounts payable, and communications. An Oversight Team and an Executive Steering Committee oversee these workstreams. Additionally, UHG employs a governance model to guide the relationship, set priorities and make critical decisions.
Will I receive credit for my years of service with LHC Group? And, if I previously worked at a UnitedHealth Group company, will I receive credit for that service? Yes, to both. You will receive credit for your combined years of employment at LHC Group and any prior years of employment with UnitedHealth Group and any of its affiliates.
Where is UnitedHealth Group headquartered? UnitedHealth Group's headquarters are in Eden Prairie, Minnesota, near Minneapolis. Team members are based in company facilities and work remotely around the world.
Is there a giving program? Yes, you will have access to the United for Giving program after integration. Year-round, UnitedHealth Group matches your financial donations to eligible charities, dollar for dollar. You can also earn $10 in rewards for every hour of volunteer time tracked in the Giving tool, up to $500 (50 hours) annually. These rewards can be donated to the eligible charities of your choice.
Does UnitedHealth Group offer other charitable contribution programs? Yes, the United for Each Other program was created to assist employees who are facing severe economic and personal hardship following a natural disaster. The program is funded primarily by charitable contributions from employees across the enterprise. Anyone may donate funds to support colleagues who have been impacted by a natural disaster or other qualifying catastrophic event. A donor does not have to be a UnitedHealth Group employee to contribute, and no donation is too small. You will have access to the United for Each Other program after integration.
What is people integration and when is it happening? People integration is tentatively scheduled for Dec. 14, 2025. This is when you’ll transition to UnitedHealth Group people (HR) and payroll systems. It’s also referred to as the payroll integration date or simply the transition date. Integration and transition are used interchangeably and applies to W2 employees only, not contractors.
What are some tasks I’ll need to complete as part of people integration? You will need to consent to a background check, submit a new Form I-9, enroll in elected benefits, and complete several other onboarding tasks, including enrolling in direct deposit and completing new tax withholding forms. Throughout the people integration period, you will receive regular communications that will update you on the things you’ll need to complete and outline important deadlines.
When will I know more about people integration and what I need to do? You will start to receive regular updates from Optum in September, which will keep you informed about steps you need to take. You’ll also be invited to live virtual benefits education and orientation sessions in the months leading up to integration to learn more about the benefits, systems and tools you’ll have access to once you integrate. IMPORTANT: Please check your LHC-issued email daily and read all integration updates so you are prepared to take action when needed.
Do I have to complete a background check? Yes. One of the steps UnitedHealth Group takes to maintain our reputation for honesty and integrity and keep our workplaces safe is to conduct background investigations into all new team members, including those in acquired companies who are integrating. Your background check will be completed in partnership with our third-party vendor, HireRight. It will include a criminal search, verification of your Social Security Number, a national sex offender registry check, a healthcare sanctions check and any other necessary screens, such as a licensure check if your role requires a professional license or certification.
What happens if something is flagged in my background check? Any adverse reports (criminal records, etc.) on a background check are reviewed by an internal team to determine the next steps. If any action is required, someone from the integration team will contact you directly.
Do I have to complete a new Form I-9? Company policy — in alignment with federal law — requires all U.S. team members who transition to new legal employer complete a new Form I-9. You'll hear more about the completion process before you need to take action. To prepare, review the Lists of Acceptable Documents and begin to gather your original and unexpired documentation.
How does UnitedHealth Group onboard team members, including those who integrate into the enterprise? You will receive an email invitation to log in to New Employee Connect (NEC), a self-service onboarding tool, to securely access employee information and complete certain tasks prior to the Dec. 14, 2025, integration. You’ll also be invited to a virtual orientation session so you can learn more about the systems and tools used across the enterprise.

I’m on a continuous leave of absence during October and November. What happens with my employment transition letter? Your personalized letter will be sent to your personal email. You will also receive an email with additional direction around this period, including how to enroll in benefits and direct deposit if your leave extends into integration.
Will my benefits change? At the time of People integration, you’ll be eligible for UnitedHealth Group-sponsored plans. Until then, your benefits — including your medical, dental and vision plans — will remain as is.
Is there benefits coverage for domestic partners? Yes. We offer domestic partner (same and opposite gender) coverage for most health, life and voluntary benefits.
Will I need to provide dependent verification documentation for my enrolled dependents? Yes. Dependent verification documentation is required to confirm dependent eligibility for coverage. Once you submit the documentation, it takes up to three business days for a determination if you upload your documents to the Benefits Site when enrolling, seven business days if you fax it, and up to 21 business days if you mail it. You'll receive a letter at your home address explaining the process after you enroll at least one dependent in medical, dental or vision.
Will my current beneficiaries automatically transfer? No, none of this information transfers automatically.
How will I know when it’s time to enroll? The day after integration, you will receive a benefits enrollment message to your work email address announcing your 30-day enrollment period. The email will include a link to the Benefits Site, which is where you’ll go to make your elections, and contact information if you have questions.
How do I enroll in benefits if I go out on a leave of absence before, or during the initial enrollment period? You enroll Dec. 15–Jan. 13, which is the same timeframe as your colleagues. If you don’t enroll during your initial enrollment period, you won’t have medical, dental or vision coverage, or most other elected benefits such as Critical Illness or Accident Insurance, for the 2026 calendar year. You won’t be able to enroll in benefits until the 2027 Open Enrollment period, unless you have a qualified status change such as marriage, divorce, or birth/adoption of a child.
What happens if I do not enroll in benefits during the initial enrollment period? If you don’t enroll during your initial enrollment period, you will not have coverage under a medical, dental or vision plan, or most other voluntary benefits you need to elect (i.e., Critical Illness or Accident Insurance) for the 2026 calendar year. Unless you have a qualified status change such as marriage, divorce or birth/adoption of a child, your next opportunity to enroll will be during the enterprise-wide Open Enrollment period, which typically takes place late fall, for coverage starting Jan. 1, 2027.
What if I’d rather have coverage under my spouse or domestic partner’s benefits and their open enrollment period has already ended? Ordinarily, you may not change elections mid-year. However, the IRS allows, and most plans permit, exceptions to this rule for certain qualifying life events, including a “change in coverage under another employer plan.” The Dec. 14 integration date is typically considered a qualifying event by other employer-sponsored plans. Please contact your spouse or domestic partner’s benefits administrator to confirm.
When do deductions for elected benefits (medical, dental, vision, etc.) start? The date you enroll in benefits affects when premiums start being deducted from your paycheck. Premiums are owed for each paycheck in the month your coverage begins, regardless of when during the enrollment period you enroll. No more than two deductions will be taken from a single paycheck.
Besides premiums, will I see any other benefits deductions on my paycheck? You might. Because you automatically receive Basic Life Insurance and AD&D coverage effective on your transition date, you are taxed on the value of the company-paid premiums above $50,000. This is called imputed income. If applicable, you will start to see imputed income deductions on your first UnitedHealth Group-administered paycheck. Depending on your integration date, you may see a double deduction on your first check and a single deduction going forward.
How do I access the United Well-being Site? Visit unitedwellbeing.uhg.com. Keep in mind the site is intended to show a summary of what’s offered, so certain benefits and plans may not be available to you. Additionally, you will not be able to access certain links until you complete the first-time login process after integration.
After I enroll, when can providers verify my insurance coverage? After you enroll, network providers will typically be able to verify your insurance coverage within three business days. ID cards are not required to receive services.
When will I receive my medical, dental, and/or vision ID cards? You should receive your medical and dental member ID cards at your home address approximately two weeks after you enroll. A vision ID card will not be issued. You can also access your medical and dental member ID cards on the myuhc.com website or UnitedHealthcare app several days after you complete your enrollment.
What if I need care before my new benefits can be verified? You will be reimbursed according to your new plan’s provisions. You’ll pay out-of-pocket (or the non-discounted amount for pharmacy) and submit your receipt(s) for reimbursement once your medical coverage is verified. If a true emergency occurs and you are taken to the nearest hospital, you will be admitted even if the hospital cannot verify your medical coverage. Even if the facility is not in the network, services will be covered at the network level until you can be relocated to a network facility. You can ask the facility to submit the claim after you’ve enrolled in benefits.
What types of medical plans are offered? We offer two types of plans, and you can choose the one that best fits your needs and how you expect to use health care. The plans offered to you are based on your home ZIP code.
I have an upcoming procedure that is scheduled for after integration. How does integration impact that? After you enroll in a UnitedHealth Group-sponsored medical plan, you will need to notify your provider of your new medical plan and work with them to reestablish any prior authorizations. The cost share will be based on the provisions of your newly elected medical plan. If your provider is no longer in-network under your newly elected plan, you can request and submit a Transition of Care application, if eligible, to see if they will extend your coverage at network rates for a limited time.
How does integration impact access to prescriptions? Prescription coverage will continue to be provided through Optum Rx, but you will no longer be able to participate in CVS Saver Plus program for maintenance medications. Instead, you will have access to Optum Home Delivery, which utilizes the Mail Service Saver Plus (MSSP) Program and the Standard Select Walgreens network. In December, Optum Rx will mail an announcement to members’ homes introducing the new service. Beginning Jan. 1, 2026, any covered home delivery prescriptions will be transitioned.
After you enroll in a new medical plan, you will need to reestablish any prior-authorizations and the cost share will be based on the provisions of your newly elected medical plan. If you currently use an Optum Rx network retail pharmacy, present your new medical ID card to have your covered prescription refilled. For details on pharmacy coverage under the new plans, please refer to Health and Pharmacy Tools.
Are Flexible Spendings Accounts (FSA) offered under the new benefits offering? Yes, we offer both Health Care and Dependent Care FSAs.
What’s a Health Savings Account (HSA)? If you enroll in an HSA-eligible plan, an HSA will be automatically opened for you at Optum Bank. The company contributes to you HSA on a biweekly basis (the amount is based on who you cover – you only, you + spouse or domestic partner, or you + family) and you can also put your own money into the account by making pretax payroll contributions, up to IRS limits. You own your HSA, which means it’s yours to keep even if you switch to another medical plan, change jobs or retire.
What will happen to my current Health Savings Account (HSA) with LHC Group? You can continue to use your funds to pay for eligible expenses after integration, but you won’t be able to make new contributions to the account through payroll. If you elect an HSA-eligible plan after integration, an HSA account is opened for you at Optum Bank. You may choose to transfer or rollover your funds and consolidate them in the Optum Bank account. Please contact your legacy custodian and Optum Bank post-integration for more information about these processes and to avoid any tax penalties.
I currently have an HSA with another administrator. Can I transfer the funds to my newly-opened HSA with Optum Bank? Yes, but you must wait until a couple of pay cycles after you enroll in an HSA-eligible plan to initiate the transfer. Optum Bank will have the information you need (including which form to use, whether there’s a transfer fee, etc.), but you should also contact your current HSA administrator to learn what steps you need to take to close out the account. Transferring funds to Optum Bank is optional – your existing account will stay with you and you can continue to use the funds to pay for eligible expenses after integration, but you’ll be responsible for the monthly administrative fee.
What will happen to my current HSA if I don’t enroll in an HSA-eligible medical plan under the new benefits package? You can continue to use your funds to pay for eligible expenses, but you won’t be able to make new contributions to the account through payroll.

What happens to my Flexible Spending Account (FSA) with LHC Group? The plan year for your current FSA ends on Sept. 30. All previously rolled over funds and funds contributed to your FSA between Oct. 1, 2024, and Sept. 30, 2025, MUST be spent before Dec. 15, 2025 (although you have until Dec. 31 to file any relevant claims). Any funds that you do not spend will be forfeited. For information about your FSA – including eligible expenses and reimbursement timing – contact your current FSA administrator, Highmark Blue Cross Blue Shield at 888-697-8722.
I have an FSA with LHC Group and need to spend down my contributions. What can I use the funds for?
Is there a transportation program? Yes. The Commuter Expense Reimbursement Account (CERA) is a flexible spending account that lets you set aside pretax dollars to pay for eligible commuter expenses, including qualified parking and public transportation. When you enroll in CERA, pretax payroll deductions are taken from your paycheck to help you lower your overall taxable income. Every pretax dollar you contribute to this account decreases your taxable income by the same amount. You can contribute up to $325 per month for parking and transit accounts. If you enroll by the 10th of the month, your participation begins on the first day of the following month. CERA deductions are taken from your second paycheck each month.
I will be turning 65 soon. Do I need to enroll and start paying for Medicare? You will want to compare coverage between Medicare and your current health care coverage when you turn 65. If you defer your Social Security benefits, you will NOT automatically be enrolled in Medicare Parts A & B and can choose to defer enrollment until later. You can also wait to enroll in Medicare Part B until you leave the UnitedHealth Group enterprise without having to pay a penalty.
If I have Medicare Part A coverage, does my medical coverage through the UnitedHealth Group enterprise pay first or Medicare? If you are employed (regardless how old you are), your employer-sponsored medical coverage is primary, and Medicare Part A is secondary until you leave the company.
My spouse, who is 67, is enrolling in a Medicare Supplemental Plan and planning to enroll in Medicare Part B. Medicare needs a “L564-Request for Employment Information” form, so my spouse can avoid a penalty and enroll now. Who do I contact? After integration and once you have enrolled in a medical plan, you can call the respective call center and request they complete a CMS_L564 Form for your spouse. The representative will complete the form and mail the original to your home address in approximately 10 business days. If you need the form before integration, please contact your current benefits administrator.
I have questions on my current LHC Group 401(k) and the transition from Voya to Fidelity. This is separate from your Dec. 14 people integration. You should have received a transition guide with more information on Friday, Sept. 19.
Will a 401(k) plan be offered? Yes, you will be eligible for a new 401(k) plan with an employer matching contribution. You will be automatically enrolled at a pretax contribution rate of 3% of eligible pay as of your second UnitedHealth Group-administered paycheck, but you can elect a different contribution level or opt out from contributing. You can contribute 1% to 80% of your eligible pay on a pretax or Roth after-tax basis up to the IRS annual limit ($24,500 in 2026). If you will be age 50 or older in 2026, you can contribute an additional $8,000 during the calendar year (or defer up to $11,250 when you are aged 60–63). Please note: You will not be able to access your new 401(k) account at Fidelity, the Plan's record keeper, until the Tuesday after integration.
Can I start my 401(k) contributions with my first paycheck after integration instead of waiting to be automatically enrolled as of my second paycheck? Yes. If you want to start contributing as of your first UnitedHealth Group-administered paycheck on Jan. 2, you need to take action by 3 p.m. CT on Dec. 24.
When will I receive matching contributions under the new 401(k) plan? You’re eligible to receive company matching contributions once you’re credited with one year of service (you’ll receive credit for your service at your current organization and any prior years of service with UnitedHealth Group and any of its affiliates). To receive the full employer match, you must contribute at least 6% of your eligible pay to your 401(k) each pay period.
Will there be a lapse in 401(k) contributions during the changeover? Post-integration, if you do nothing, you’ll be automatically enrolled in the new 401(k) plan starting on your second UnitedHealth Group-administered paycheck on Jan. 16. If you want to start contributing as of your first UnitedHealth Group-administered paycheck on Jan. 2, you need to take action by 3 p.m. CT on Dec. 24, which will be outlined on the Onboarding Checklist you’ll receive prior to integration.
Will my current 401(k) beneficiaries and contribution rate automatically transfer as part of the changeover? No, none of this information transfers automatically.
When will I be vested in UnitedHealth Group 401(k) plan matching contributions? Matching contributions are 100% vested after you’ve completed two years of service (you’ll receive credit for your service at your current organization and any prior year of service with UnitedHealth Group and any of its affiliates).
What will happen to my current 401(k) account at LHC Group? At the time of integration, your current 401(k) plan will be frozen. This means:
What will happen to the loan from my current 401(k) account? If you have a loan from your LHC Group 401(k) plan, deductions will continue to be withheld from your UnitedHealth Group-administered paycheck after integration and deposited into your LHC Group 401(k) plan account. When the plans merge in the future, your loan will automatically transfer to the new 401(k) plan along with the rest of your account. You do not need to take any action. Please note: If you take out a new loan, we will look across your current 401(k) plan with LHC Group and the new 401(k) plan to ensure you do not exceed IRS maximum loan limits.
Does UnitedHealth Group offer an Employee Stock Purchase Plan (ESPP)? Yes, for eligible team members. If you are regularly scheduled to work at least 20 hours per week, or more than five months per year, you can buy UnitedHealth Group common stock at a 10% discount. You can contribute 1-10% of your base salary, up to certain plan limits, through payroll deduction. There are two enrollment periods each year and eligible team members will receive an email invitation from Fidelity when it's time to enroll. If eligible, you can start participating in the first purchase period of 2026 and will have an opportunity to enroll Dec. 18–23.
How much stock can I purchase each year through the ESPP? The IRS limits the total amount of stock you can purchase each calendar year to $25,000 based on the fair market value (FMV) of the stock. The FMV is determined using the stock closing price on the first day of each purchase period. Within these IRS limits, you can purchase up to 1,000 shares of stock each purchase period.
Does UnitedHealth Group reimburse team members for fees associated with required licenses and certifications? Yes, when a license or certification is required by the company as a condition of employment, 100% of the license or certification is reimbursable. Team members must pay these fees and request reimbursement through the appropriate expense reimbursement process.
Does UnitedHealth Group offer banking services? Yes, you have access to the UnitedHealth Group Credit Union. This full-service financial institution offers accessible, not-for-profit banking services to help you and your family achieve your goals and build financial confidence. Some of the benefits of banking with the credit union are outlined below.
Can I join the UnitedHealth Group Credit Union now or do I need to wait until after people integration? Yes, you and your family members are eligible to join at any time. To learn more or become a member, visit uhgcu.org/newhire.
Is tuition reimbursement offered? Yes. When you have been employed for at least six months (including your service at your current organization) and are scheduled to work 20 or more hours per week (contractors, temporary employees and employees on a leave of absence are not eligible), you may qualify for up to $5,250 of education reimbursement per calendar year for approved coursework in accredited programs.
What is UnitedHealth Group’s approach to paid time away from work? We offer team members a Paid Time Off (PTO) program that combines vacation and sick leave into a single bank of time to use when you’re away from work, with your manager's approval and in consideration of business needs.
Are sick days offered? Typically, no – we combine vacation time, personal time and sick time into our PTO bank. However, employees not eligible for PTO may be provided with paid sick and safe time if mandated by city, county and state legislation.
I already received approval to take time away from work, but it’s scheduled for after integration. Can I still that time away? Yes, as long as it’s in accordance with the company PTO policy and your manager approves.
Will my court-ordered payment payroll deductions (e.g., child support) continue after integration? New orders will be sent to our payroll. This may cause a delay in the deductions. If you are concerned about a payment being missed, you should contact the issuing agency to discuss the transfer of court-order information to our payroll.
What holidays does UnitedHealth Group observe? We observe the following eight holidays. Generally, holidays that fall on a Saturday will be observed on the preceding Friday, and holidays that fall on a Sunday will be observed on the following Monday, as outlined on the holiday calendar.

Does UnitedHealth Group offer floating holidays? Yes, you will be granted one floating holiday each calendar year (starting in 2026) to take paid time off to celebrate a day that’s meaningful to you. The number of hours you receive is equivalent to 1/5 of the number of your standard weekly hours. For example, if you normally work 30 hours per work week, you will receive 6 hours and if you normally work 40 hours per work week, you will receive 8 hours.
How will I access my paystub post-transition? You'll be able to view your UnitedHealth Group-administered paystubs in MyHR, our HR transactions system, the Wednesday before each pay date. It will also be visible in the UnitedHealth Group employee app.
How will transition affect my W-2 and other taxes withheld from my pay? You will need to complete new tax withholding forms as your current tax information won’t automatically transfer. You will not receive a W-2 from the UnitedHealth Group enterprise for 2025 since you won’t receive your first payslip until 2026.
Does UnitedHealth Group offer direct deposit? Yes. You'll need to enroll in direct deposit in our system — your current direct deposit information will not transfer automatically.
Will my tax withholding information transfer to the UnitedHealth Group enterprise? No, your tax withholding details will not transfer. You will need to complete a new W-4 form as part of your onboarding activities. Please note: The W-4 form was redesigned in 2020. It no longer uses withholding allowances and instead asks for specific dollar amounts to help determine your withholding. For more information on the updated form, visit the IRS W-4 FAQs.
In what increments can I take PTO after transition? In general, non-exempt team members can take PTO in one-hour increments and exempt team members can take PTO in full- or half-day increments. There are some exceptions for those on approved intermittent Family and Medical Leave, or if required by law. You'll have access to the full policy after integration.
Can I borrow PTO before it’s been granted within the UnitedHealth Group enterprise? Yes. You can borrow up to one week’s worth of your standard hours (up to 40) from your future PTO with your leader’s approval.
When can I expect my unused PTO at LHC Group to transition to my new PTO bank within the UnitedHealth Group enterprise? Transferring hours may not be visible until your second UnitedHealth Group-administered payslip on Jan. 16.
When will I begin receiving PTO with the UnitedHealth Group enterprise? You’ll receive your first PTO grant on Jan. 2, 2026, the date of your first UnitedHealth Group-administered payslip, and every biweekly pay period thereafter.
If I work less than 40 hours per week, am I eligible for PTO with the UnitedHealth Group enterprise? If you work 20 to 39 hours per week, you’re eligible for PTO on a prorated basis. If you work less than 20 hours per week or are a temporary employee, you are not eligible for PTO.
How much PTO may I carry over from one calendar year to the next under the new policy? You can carry over up to one week’s worth of your standard hours (up to 40 hours) from year to year, unless otherwise required by state law. Any unused PTO that exceeds these limits will be forfeited at year-end.
How can I view my PTO balance after integration? You can view it in our HR transactions system and on your pay stub. Any PTO that transitions from your current organization will be visible starting with your second UnitedHealth Group-administered payslip.

I reside in Massachusetts. What happens to my LHC Group sick time and PTO at integration? Your full sick time balance will transfer to the Disability Reserve Bank (DRB) for use during protected leave. Up to 40 hours of your PTO, if available, will move to your new PTO bank. If you have less than 40 hours of PTO, we’ll use your DRB balance to bring your PTO total up to 40 hours.

If you reside outside of Massachusetts, any Safe and Sick time you accrued with LHC Group will transfer to your new PTO bank at the time of integration. You can use these hours to take time away from work as needed, in line with the new PTO policy.

What holidays will LHC Group recognize in 2025 under the new holiday policy? Because integration occurs on Dec. 14, the only company-recognized holiday for 2025 is Christmas Day, which is observed on Thursday, Dec. 25. Starting in 2026, LHC Group will align to the full UnitedHealth Group holiday calendar.
What does it mean to agree to the transition letter in NEC? By agreeing to the transition letter in NEC, you acknowledge that you agree to the terms and conditions outlined in the letter. In order to move forward with the onboarding process, you must agree to the letter and fully complete your NEC Welcome steps.
What if my name is incorrect on my transition letter? If your legal first and/or last name are incorrect, please fill out this form to let us know. Middle names/middle initials are not included.
What if I have questions about the position or compensation information in my transition letter? Please talk with your manager.
Do I need to complete the required steps for the background check in order to integrate? Yes, if you do not complete the required steps in a timely manner you will not be allowed to integrate.

What information should I provide if I am required to have my professional license/certification verified? When completing your background check profile in HireRight, you will need to include your license/certification number, the state in which the license is held, and the licensing organization’s name. You only need to provide the license information if it’s relevant to your current position.
I’m on PTO and will be unable to complete my transition letter and background check process tasks within two business days. What do I do? Please complete it immediately upon your return. Successfully completing and clearing a background check is required to remain eligible for transition and continued employment, so it’s critical you take quick action as soon as you return.
Why do I need to complete the SSA-89 Form? The form is required on a case-by-case basis to assist in verifying your Social Security Number (SSN), which is something we can’t do through automation. You may have been randomly selected or there might be a discrepancy between your name, date of birth and/or your SSN. If you need to change your name in HireRight so it matches your Social Security Card, please fill out the form to let us know and we’ll send you a new HireRight invitation.
I need to complete the SSA-89 Form in HireRight and need more support. Where do I go? Email HireRight at customerservice@hireright.com, call them at 888-417-5040 or visit their Help Center. Additionally, you can send questions to the Talent Acquisition team through the form.
How can I upload my completed SSA-89 Form? It can be uploaded directly within HireRight (click the blue login button in your original email from HireRight to access the tool) or securely email the forms to documentation@hireright.com. The Social Security Administration (SSA) has strict guidelines, so please print the form(s), use a black pen to fill out your information, and ensure the paperwork is free from smudges or folds. If information is missing and/or not in a format deemed acceptable by the SSA, HireRight will request the forms again.
Can I electronically sign the SSA-89 Form? No, forms must be printed and physically signed using a wet signature (must be handwritten and non-electronic).
What happens if I do not return the SSA-89 Form in an acceptable format prior to my integration date? You will not be cleared to integrate.

Why do I need to sign the Arbitration Policy Acknowledgement Form? All employees are required to sign the form as a condition of employment. UnitedHealth Group believes that workplace concerns are best resolved through open and candid discussions and encourages early identification and dialogue to facilitate resolution. Signing the form helps ensure that employment-related disputes are resolved through a private, efficient, and less formal process than going to court.
