What’s People Integration? It is the timeframe in which you will transition to UHG Human Resources systems and programs, including payroll, benefits, rewards and recognition programs, and paid time off (PTO) and holidays. It’s also referred to as the payroll integration date or simply the transition date. We will use integration and transition interchangeably.
When is People Integration happening? People Integration is scheduled for Dec. 14, 2025.
How are integration decisions made? The engagement model between Optum and Atrius Health includes work teams with representation from Optum and Atrius Health to ensure good collaboration and partnership. In addition, a governance model has been established to guide that process, define priorities, and make critical decisions. An Oversight Team and Executive Steering Committee is in place that also includes representation from both organizations.
Will I receive credit for my years of service with Atrius Health? And, if I previously worked at a UnitedHealth Group company, will I receive credit for that service? Yes, to both. You will receive credit for your combined years of employment at Atrius Health and any prior years of employment with UnitedHealth Group and any of its affiliates.
Will my job title change? You will have a title that corresponds with your internal job code which identifies your position in the UnitedHealth Group system. Some people will continue to use their current title for business purposes (in their email signature, LinkedIn, on presentations, etc.). Business titles may change in the future.
How will integration impact our work schedules, work locations and the work we do on a daily basis (working on holidays, office days, part-time vs. full-time, overtime opportunities)? These are business decisions that are made locally and are not part of the People Integration.
Will I need to relocate? No.
Will my email address and/or phone number change? For most team members, no, although you may receive a new email and/or phone number at some point in the future.
Will there be a change to our dress code? No. Local leadership establishes dress code guidelines.
What is not included in the People Integration? It does not include changes to technology, branding or finance, although there are other integration workstreams that are addressing these items.
When will I know more about the People Integration and what I need to do? Beginning in May, you will begin to receive regular communication from your local leadership and transition update emails from Optum to keep you informed about steps you need to take. You’ll also be invited to live virtual benefits education and orientation sessions closer to your integration to learn more about the benefits, systems, and tools you’ll have access to once you integrate.
When will we be fully integrated into Optum and what does that mean? Integration with Optum is a process and a journey — carefully and thoughtfully planned and executed. There are five phases of integration. You can learn more about each phase here. As integration plans are defined, we will provide information to ensure transparency and clarity on what to expect.
Why are physicians and APCs aligned to a professional corporation (PC) instead of Optum Services Inc., like other staff? Corporate practice of medicine (CPOM) prohibits corporations from employing individuals who deliver care. Its laws were designed to prevent the commercialization of medicine and allow individuals who deliver care to focus solely on a patient’s best interest. To comply with CPOM, providers within the UnitedHealth Group enterprise are employed by a PC so they can be governed and guided by a separate set of policies and protocols specifically built to support the practice and their role as a provider.
What is Optum Partner Services? Optum Partner Services (OPS) is the name given to a suite of people team (HR) and benefit services designed to support physicians, APCs and other provider team members who are part of an Optum professional corporation (PC). OPS uses UnitedHealth Group systems and infrastructure to provide these services to providers, but it is not a legal entity and does not employ them.
Does Optum/UnitedHealth Group allow for remote work outside of the U.S.? No.
If I'm on leave at integration, what do I need to do and how will I get information? The only thing you need to do is enroll in benefits during the same initial enrollment period as the rest of Atrius Health. You'll do everything else once you've provided your return-to-work date or returned to work. If you're on leave during people integration, expect to receive a letter outlining important information about the integration, and you can always reach out to your manager or HR department if you have questions.
Who can verify my employment now and post-integration? If you need your employment verified now through people integration, follow your current process. After people integration, you’ll follow the UnitedHealth Group process. UnitedHealth Group uses The Work Number, a service of Equifax, to provide automated employment and income verifications, letters for immigration and visas, and external job references. After your integration date, search the UnitedHealth Group intranet, Sparq, for “The Work Number” for more information.
What if I am a member of the MNA or SEIU unions? We are working collaboratively with each union to ensure we follow our collectively bargained agreements while we integrate systems, policies, and benefits for our employees.
Why are we integrating on Dec. 14 and not Dec. 31? Atrius' payroll cycle matches that of UnitedHealth Group. Making the last day under Atrius Dec. 13 allows for the final paycheck from Atrius to fall in 2025, on Dec. 19, 2025. Starting at UHG on Dec. 14 makes the first UHG paycheck fall into 2026, on Jan. 2, 2026. This allows for a clean cut between the two years and year end processes, and team members will get one W2 for 2025 from Atrius and one W2 from UHG for 2026.
What learning and development opportunities exist for clinicians with Optum? There are many great professional and leadership development opportunities available to clinicians. The Optum Office for Provider Advancement offers some programs that are available right away, such as the Grove leadership essentials program for those who are newer in their role and the Climb program for medical directors, as well as live webinars and on-demand learning. There are also other opportunities such as the Optum Physician Partnership Program and a host of accredited programming through Optum Health Education, which is available at any time.
What is the Optum Physician Partnership Program? The Optum Physician Partnership Program is designed to reward, recognize, and develop physicians in all aspects of care throughout Optum. The program brings physicians together for learning and engagement opportunities, including an annual forum. In addition to offering multiple career tracks and levels of participation for physicians, there is also a newer track for advanced practice clinicians (Optum Partnership Program for APCs).
How will UnitedHealth Group and Optum support my career growth and professional development? UnitedHealth Group is a Fortune 5 organization with team members across the globe. Post-integration, you'll have access to several career development resources, opportunities and a larger network and you'll be able to map your own career journey with support from your manager.
Does UnitedHealth Group reimburse team members for fees associated with required licenses and certifications? When a license or certification is required by the company as a condition of employment, 100% of the license or certification is reimbursable. Team members must pay these fees and request reimbursement through the appropriate expense reimbursement process.
Is tuition reimbursement offered? Yes. When you have been employed for at least six months (including your service at Atrius Health) and are scheduled to work 20 or more hours per week (contractors, temporary employees, and employees on a leave of absence are not eligible), you may qualify for up to $5,250 of education reimbursement per calendar year for approved coursework in accredited programs.
Please note that any employees who are represented by a union should refer to their collective bargaining agreement on any topics outlined below.
What will happen to the PTO I have already accrued at Atrius Health? Atrius Health staff will transition up to 40 hours of accrued but unused PTO. Providers who currently receive discretionary time will be granted 40 hours of PTO.
What if I have more than 40 hours of accrued but unused PTO? Atrius Health staff (excludes providers who currently receive discretionary time) will receive a PTO payout for any accrued but unused hours they’ve accumulated over 40. For example: If you have 50 PTO hours at the time of integration, 40 will transition, and you will receive a PTO payout for the remaining 10 hours.
When will I receive my PTO payout? On your final Atrius-administered paycheck (Dec. 19, 2025).
If I’m a provider who receives discretionary time, will I receive a PTO payout? No, only staff members with accrued but unused PTO hours totaling more than 40 will receive a payout at integration. Discretionary time is untracked and does not accrue an available balance in the same way that PTO does, therefore there are no hours to be paid out.
Will there be a PTO payout every calendar year or is this a one time offer for integration? The PTO payout happens for eligible Atrius Health staff (excludes providers who currently receive discretionary time) at integration and then when an employee leaves the company (if required by state law.) Each year only a standard week of hours, with a maximum of 40, will carry over to the next year.
Are the 40 hours of PTO transitioning over prorated for part time staff? The PTO transition balance is up to 40 hours of accrued but unused time for all non-union employees, regardless of their standard hours.
Does this mean that staff will have 40 hours of PTO going into the last two weeks of December? Your transitioning PTO hours will most likely not be in your new Optum PTO bank until your second UHG paycheck (Jan. 16, 2026). In the meantime and with your manager’s approval, you can borrow up to one week of your standard hours, up to a maximum of 40 hours before the PTO has been granted.
When can I expect my unused PTO at Atrius Health to transition to my new Optum PTO bank? Transitioning PTO hours are generally not visible until your second UnitedHealth Group-administered paycheck on Jan. 16, 2026.
How much PTO may I carry over from one calendar year to the next? If you do not use all your accrued but unused PTO within a calendar year, you may carry over up to one week of your standard work hours, with a maximum limit of 40 hours, into the following year.
What holidays does UnitedHealth Group observe? We observe the following eight holidays. Generally, holidays that fall on a Saturday will be observed on the preceding Friday, and holidays that fall on a Sunday will be observed on the following Monday.

What additional compensation will I receive if I work on a company holiday? Non-exempt employees who work on a company holiday will receive holiday pay based on regularly scheduled hours, as well as pay at time and one half for all hours worked. Company holiday hours paid is based on 1/5 of your standard hours (for example, if your standard hours are 40/week, you will receive 8 hours of holiday pay).
Does UnitedHealth Group offer floating holidays? Yes, you will be granted one floating holiday each calendar year to take paid time off to celebrate a day that’s meaningful to you. The number of hours you receive is equivalent to 1/5 of the number of your standard weekly hours. For example, if you normally work 30 hours per work week, you will receive 6 hours and if you normally work 40 hours per work week, you will receive 8 hours. Because your integration takes place in December, you are not eligible to receive a Floating Personal Holiday grant until the 2026 calendar year.
What happens to my Extended Sick Leave (ESL) benefit? Atrius Health team members will no longer accrue ESL, nor have the option of converting ESL days to personal days. While ESL will no longer be accrued, existing ESL balances will transition to the UHG Disability Reserve Bank. These hours can be used during an approved Leave of Absence for your own illness or to care for your child, parent, spouse or domestic partner who has a serious health condition.
I have a service anniversary in Q4 2025. When will my ESL days convert to personal days? Any Atrius Health team members with a service anniversary in Q4 2025 will have their two ESL days converted into personal days in September 2025, rather than at the time of their anniversary date. This change allows for adequate time to use the days prior to integration on Dec. 14, 2025. For any team members represented by the MNA or SEIU: those with a Q4 service anniversary will have their two sick days converted into personal days during their anniversary month.
I need to take a leave next year in 2026, how is my ESL going to work with the Disability Reserve Bank? The Disability Reserve Bank is available to be used during an approved Leave of Absence for your own illness or to care for your child, parent, spouse or domestic partner who has a serious health condition. Once the leave administrator approves your Short-Term Disability (STD), Long-Term Disability (LTD) or Family Leave of Absence, the leave administrator will automatically apply available Disability Reserve Bank time as appropriate. These hours will continue to be applied automatically throughout the duration of your approved leave until the hours are exhausted, or you return to work. Disability Reserve Bank time will be used before any Paid Time Off (PTO).
Can my converted ESL time be donated similar to PTO? No, the time in your UHG Disability Reserve Bank cannot be donated like PTO can.
What types of leaves of absence does UnitedHealth Group offer? We care about your well-being and know you may need to take time off for major life events. To support you, we offer several leaves, including FMLA, Paid Caregiver, Paid Parental and Military. We also have a Bereavement Time Off Policy, which allows you paid time off for a pregnancy loss or the death of an immediate family member (i.e., spouse, domestic partner, child, sibling, parent, grandparent, etc.). You will have access to the full policies after integration, which outline eligibility and specifics. Case-by-case needs can be discussed and will be handled according to policy, state guidelines and other factors.
Members of the Massachusetts Nursing Association (MNA) and Service Employees International Union (SEIU) should refer to their collective bargaining agreements for specifics on paid time off and recognized paid holidays.
Can I take time off without pay? If you take planned or unplanned time off and you have PTO available, you must use PTO to cover the absence. The only exception to mandatory use of PTO would be if your absence is covered by one of the leave of absence polices that allow you to choose whether or not to apply PTO to the absence (e.g., Family and Medical Leave). Personal leave is unpaid, however, you must use any time away from work balances to be paid for as many hours of your personal leave as possible
Is the PTO pay out at 100%? The payment of accrued but unused PTO hours, that exceeds the 40-hour transition amount, will be paid at an employee’s straight time regular rate.
If an employee works a 10-hour shift, does holiday pay cover the 10 hours or only 8? Regarding non-exempt employees, company holiday pay is based on each employee's hourly rate and number of weekly scheduled hours. U.S. holiday hours for each employee will be reflected on timesheets based on one-fifth of the employee's standard weekly hours. For example, an employee whose normal schedule is 35 hours per work week will receive seven (7) hours of pay at their regular hourly rate for a holiday. Eligible employees are paid for the holiday even though it may not be a scheduled workday.
How is the PTO payout in December going to be taxed? The PTO payout will be taxed as regular income.
Will the PTO payout include all regular deductions from my paycheck? The PTO payout will include all regular taxes and retirement deductions.
Regarding the PTO payout, will we have enough notice to adjust our 401(k) contributions? Yes.
What is UnitedHealth Group’s approach to paid time away from work? We offer team members a Paid Time Off (PTO) program that combines vacation and sick leave into a single bank of time to use when you’re away from work, with your manager's approval and in consideration of business needs.
Are separate sick days offered? Our PTO bank includes vacation time, personal time and sick time. However, employees not eligible for PTO may be provided with paid sick and safe time if mandated by city, county and state legislation.
I already received approval to take time away from work, but it’s scheduled for after integration. Can I still that time away? Yes, as long as it’s in accordance with the company PTO policy and your manager approves. You will need to reenter the time in the new timekeeping system once you have access after integration.
In what increments can I take PTO after transition? In general, non-exempt team members can take PTO in one-hour increments and exempt team members can take PTO in full- or half-day increments. There are some exceptions for those on approved intermittent Family and Medical Leave, or if required by law. You'll have access to the full policy after integration.
When will I begin receiving PTO with the UnitedHealth Group enterprise? You’ll start accumulating PTO on Jan. 2, 2026, the date of your first UnitedHealth Group-administered paycheck, and every biweekly pay period thereafter. Additionally, you can borrow up to one week of your standard hours, up to a maximum of 40 hours, with your manager’s approval.
What is my PTO accumulation rate? Information on your PTO accumulation rate can be found here.
If I work less than 40 hours per week, am I eligible for Paid Time Off (PTO) with the UnitedHealth Group enterprise? If you work 20 to 39 hours per week, you’re eligible for PTO on a prorated basis. If you work less than 20 hours per week or are a temporary employee, you are not eligible for PTO. Please note that team members whose standard working hours are less than 20 per week/providers with less than 0.50 FTE are ineligible to accumulate PTO under the UHG policy but may be eligible for paid sick time.
How can I view my PTO balance after integration? You can view it in our HR transactions system and on your pay stub. Any PTO that transitions from your current organization will be visible starting with your second UnitedHealth Group-administered paycheck.
In the future, will I be taxed on any PTO I choose to donate to the UHG PTO Donation Bank? No, the value of your donated hours is not tax deductible.
After integration, what happens to my PTO balance if I leave the company? PTO is a grant and not earned compensation. Therefore:
Is the PTO payout upon leaving the company after integration based on the state I work in or the state I live in? PTO payout upon leaving the company is based on state you work in.
What is happening to the Atrius Health 401(k) core contribution? The Atrius Health 401(k) contribution reflecting Play Year 2024 is tentatively due for distribution in July 2025. The core contribution reflecting Plan Year 2025 has a projected payout within a similar timeframe next year.
What will happen to the loan from my current 401(k) account? If you have a loan from your Atrius Health 401(k) plan, deductions will continue to be withheld from your UnitedHealth Group-administered paycheck after integration and deposited into your Atrius Health 401(k) plan account. When the plans merge in the future, your loan will automatically transfer to the new 401(k) plan along with the rest of your account. You do not need to take any action.
Is the Optum 401(k) a Fidelity 401(k)? Yes, Fidelity is the plan’s record keeper.
When will I hear more information about Health & Wellness benefits? A transition update regarding Health & Wellness information related to integration is tentatively scheduled to be sent in early November 2025.
Will a 401(k) plan be offered? Yes, you will be eligible for a new 401(k) plan with an employer matching contribution. You will be automatically enrolled at a pretax contribution rate of 3% of eligible pay as of your second UnitedHealth Group-administered paycheck, but you can elect a different contribution level or opt out from contributing. You can contribute 1% to 80% of your eligible pay on a pretax or Roth after-tax basis up to the IRS annual limit ($24,500 in 2026). If you will be age 50 or older in 2026, you can contribute an additional $8,000 during the calendar year (or defer up to $11,250 when you are aged 60–63). Please note: You will not be able to access your new 401(k) account at Fidelity, the Plan's record keeper, until the Tuesday after integration.
Can I start my 401(k) contributions with my first paycheck after integration instead of waiting to be automatically enrolled as of my second paycheck? Yes. If you want to start contributing as of your first UnitedHealth Group-administered paycheck on Jan. 2, you need to take action by 3 p.m. CT on Dec. 24.
When will I receive matching contributions under the new 401(k) plan? You’re eligible to receive company matching contributions once you’re credited with one year of service (you’ll receive credit for your service at your current organization and any prior years of service with UnitedHealth Group and any of its affiliates). To receive the full employer match of 3.5%, you must contribute at least 6% of your eligible pay to your 401(k) each pay period:
% of eligible pay you contribute | Company match |
| 1% | $1 for each $1 you contribute |
| 2% to 6% | 50 cents for each $1 you contribute |
When will I be vested in UnitedHealth Group 401(k) plan matching contributions? Matching contributions are 100% vested after you’ve completed two years of service (you’ll receive credit for your service at your current organization and any prior year of service with UnitedHealth Group and any of its affiliates).
What will happen to my current 401(k) account at Atrius? At the time of integration, your current 401(k) plan will be frozen. This means:
Will there be a lapse in 401(k) contributions during the changeover? Post-integration, if you do nothing, you’ll be automatically enrolled in the new 401(k) plan starting on your second UnitedHealth Group-administered paycheck on Jan. 16. If you want to start contributing as of your first UnitedHealth Group-administered paycheck on Jan. 2, you need to take action by 3 p.m. CT on Dec. 24, which will be outlined on the Onboarding Checklist you’ll receive prior to integration.
Will my current 401(k) beneficiaries and contribution rate automatically transfer as part of the changeover? No, none of this information transfers automatically.
Does UnitedHealth Group offer an Employee Stock Purchase Plan (ESPP)? Yes, for eligible team members. If you are regularly scheduled to work at least 20 hours per work, or more than five months per year, you can buy UnitedHealth Group common stock at a 10% discount. You can contribute 1-10% of your base salary, up to certain plan limits, through payroll deduction. There are two enrollment periods each year and eligible team members will receive an email invitation from Fidelity when it is time to enroll. If eligible, you can start participating in the first purchase period of 2026 and will have an opportunity to enroll Dec. 18-23.
How much stock can I purchase each year through the ESPP? The IRS limits the total amount of stock you can purchase each calendar year to $25,000 based on the fair market value (FMV) of the stock. The FMV is determined using the stock closing price on the first day of each purchase period. Within these IRS limits, you can purchase up to 1,000 shares of stock each purchase period.

Is there a giving program? Yes, you will have access to the United for Giving program after transition. Year-round, we'll match your financial donations to eligible charities, dollar for dollar. We'll also donate $500 to a nonprofit of your choice after you've tracked 50 hours of volunteer time in the Giving tool.
How will payroll be processed after transition? Your payroll will be administered by UnitedHealth Group. The pay period begins on Sunday and concludes two weeks later on Saturday. In addition:
Since Atrius Health transitioned to the UHG payroll schedule in 2024, there will not be many broad changes related to payroll. Your payday, pay schedule, etc. will remain the same as it is today. You’ll mainly just be asked to update your direct deposit information, etc.
How will my paycheck be affected by the integration? UnitedHealth Group will take over administration of your paycheck at integration. You’ll receive your first UnitedHealth Group-administered paycheck on Jan. 2, 2026, covering Dec. 14–27, 2025. Since Atrius Health transitioned to the UHG payroll schedule in 2024, there will not be many broad changes related to payroll. Your payday, pay schedule, etc. will remain the same as it is today.
Will I need to complete new tax withholding forms and reenroll in direct deposit? Yes. This information will not transfer from your current organization to UnitedHealth Group. You will be notified when it’s time to complete these actions.
Will my court-ordered payment payroll deductions (e.g., child support) continue after integration? Yes. Court-ordered payments will automatically transition to our payroll.
How will I access my paystub post-transition? You'll be able to view your UnitedHealth Group-administered paystubs in MyHR, our HR transactions system, the Wednesday before each pay date. It will also be visible in the UnitedHealth Group employee app.
How will transition affect my W-2 and other taxes withheld from my paycheck? Even though you’re integrating in 2025, you won’t receive your first UnitedHealth Group-administered paycheck until 2026, so there will be no impact on your Social Security deductions, nor will you receive a W-2 from the enterprise for 2025. However, as noted above, you’ll need to complete new tax withholding forms as your current tax information won’t transfer from Atrius Health. You will receive notice before it’s time to do this.
How does UnitedHealth Group onboard team members, including those that integrate into the enterprise? You will be invited to log in to New Employee Connect (NEC), a self-service onboarding tool, to securely access employee information and complete certain tasks prior to the Dec. 14 integration. You’ll also be invited to a virtual orientation session so you can learn more about the systems and tools used across the enterprise.
Why is a background check required for integrating team members? One of the steps we take to maintain our reputation for honesty and integrity and keep our workplaces safe is to conduct background investigations on all new team members, including those in acquired companies who are integrating.
How does the background check work? Your background check will be completed in partnership with our third-party vendor, HireRight. It will include a criminal search, verification of your Social Security Number, a national sex offender registry check, a healthcare sanctions check and any other necessary screens, such as a licensure check if your role requires a professional license or certification.
Will I receive notification that I've successfully completed the background check? No, you'll only be notified if there is an issue with your background check or if additional information is needed. You can track the progress of your background check by logging into HireRight and request a copy of your background check report.
What happens if something is flagged in my background check? Any adverse reports (criminal hits, etc.) on a background check are reviewed by an internal team to determine next steps. If any action is required, someone from the integration team will contact you directly.
Only applies to staff (clinicians will sign their memo in Adobe Sign)
What does it mean to agree to the transition letter in NEC? By agreeing to the transition letter in NEC, you acknowledge that you agree to the terms and conditions outlined in the letter. In order to move forward with the onboarding process, you must agree to the letter and fully complete your NEC Welcome steps. If you don’t agree, you won’t be able to transfer to the enterprise.
What if my name is incorrect on my transition letter/memo? If your legal first and/or last name are incorrect, please email ta_integrations@uhg.com. Middle names/middle initials are not included.
What if I have questions about the position or compensation information in my transition letter/memo? Please talk with your manager.
Do I need to complete the required steps for the background check in order to integrate? Yes, if you do not complete the required steps in a timely manner you will not be allowed to integrate.
How can I access NEC if I no longer have the initial email I received? After your first login, you can access NEC at uhg.myonboarding.com. Log in with the same credentials you used during your initial login. NEC is not compatible with Internet Explorer, so please use Google Chrome, Firefox, Apple Safari or Microsoft Edge.

For 24/7 technical support, email customerservice@hireright.com or call 888-417-5040. You can also utilize the live chat feature directly in HireRight from 7 p.m. Sunday–9 p.m. Friday CT.
What information should I provide if I am required to have my professional license/certification verified? When completing your background check profile in HireRight, you will need to include your license/certification number, the state in which the license is held, and the licensing organization’s name. You only need to provide the license information if it’s relevant to your current position.
I’m on PTO and will be unable to complete my transition letter/memo and background check process tasks by the deadline. What do I do? Please let your manager know when you plan to return so they can notify the People (HR) team. HR will flag your PTO with Optum Talent Acquisition so they are aware you can’t meet the deadline. If the case you are on extended PTO, you may need to complete this process using a personal email – please email ta_integrations@uhg.com for additional instructions. Successfully completing and clearing a background check is required to remain eligible for transition and continued employment.
What if my title on my transition letter/memo is wrong? Please note that the data represented in your employment transition letter/memo reflects a specific point in time, so it may not be reflective of the latest information, for example, if you have taken a new role or moved internally since Oct. 17. Those recent changes may not appear in your transition letter/memo, but they will be reflected in the UnitedHealth Group payroll and HR systems at the time of integration.
I need to complete the SSA-89 Form in HireRight and need more support. Where do I go? You can watch this 3-minute instructional video (please disregard 1:22-1:40 – your form is prepopulated and should not be altered). You can also email HireRight at customerservice@hireright.com, call them at 888-417-5040 or visit their Help Center. Additionally, you can send questions to the Talent Acquisition team at ta_integrations@uhg.com.
Why do I need to complete the SSA-89 Form? The form is required on a case-by-case basis to assist in verifying your Social Security Number (SSN), which is something we can’t do through automation. You may have been randomly selected or there might be a discrepancy between your name, date of birth and/or your SSN. If you need to change your name in HireRight so it matches your Social Security Card, please email ta_integrations@uhg.com so they can send you a new HireRight invitation.
How can I upload my completed SSA-89 Form? It can be uploaded directly within HireRight (click the blue login button in your original email from HireRight to access the tool) or securely email the forms to documentation@hireright.com. The Social Security Administration (SSA) has strict guidelines, so please print the form(s), use a black pen to fill out your information, and ensure the paperwork is free from smudges or folds. If information is missing and/or not in a format deemed acceptable by the SSA, HireRight will request the forms again.
What happens if I do not return the SSA-89 Form in an acceptable format prior to my integration date? You will not be cleared to integrate.
Why did I get a message from HireRight saying my occupational health questionnaire was canceled? If you were asked to complete an occupational health questionnaire through our vendor, HireRight, please know that although we’re using the same vendor, the transition background screening is an entirely separate process. The occupational health questionnaire was canceled on Oct. 31, 2025, if it was not completed, which may have resulted in an email notice from HireRight referring to a background form being canceled. Do not worry, you will be prompted to complete the questionnaire during the transition background process.
Please note: Benefits FAQs below only apply to OptumCare-eligible team members. Those identified as shared services employees should refer to the email they received on Nov. 14 for details and further information.
What happens to my current coverage? If you are enrolled in your current organization’s health benefits, that coverage will end on Dec. 31, 2025.
What are my new benefit options and when can I enroll? Upon integration, you are eligible for new benefits under the UnitedHealth Group enterprise and will have an opportunity to enroll between Dec. 15 – Jan. 13, 2026. As long as you enroll during the 30-day enrollment period, your elected benefits will be effective Jan. 1, 2026. There will be no gap in coverage.
What is the company’s approach to benefits? Upon integration, you’ll be eligible for a new benefits package, which is part of a comprehensive total rewards package that includes compensation, health benefits, life and disability insurance, paid time off, paid holidays, tuition reimbursement, financial benefits, well-being programs and services, and more. Our benefits are designed to help you and your family live healthier lives, manage your health care costs and support your financial well-being.
What’s offered in the new benefits package and when do they take effect? The following chart shows a snapshot of the benefits we offer and their effective dates. In general, you’re eligible for benefits if you work at least 20 hours per week, although some benefits are available if you work less than 20 hours per week. Refer to the eligibility matrix for more information.
How will I know when it’s time to enroll? The day after integration, you will receive a benefits enrollment message to your work email address announcing your 30-day enrollment period. The email will include a link to the Benefits Site, which is where you’ll go to make your elections, and contact information if you have questions.
What happens if I do not enroll in benefits during the initial enrollment period? If you don’t enroll during your initial enrollment period, you will not have coverage under a medical, dental or vision plan, or most other voluntary benefits you need to elect (i.e., Critical Illness or Accident Insurance). Unless you have a qualified status change such as marriage, divorce or birth/adoption of a child, your next opportunity to enroll will be during the enterprise-wide Open Enrollment period, which typically takes place late fall, for coverage starting Jan. 1, 2027.
What if I’d rather have coverage under my spouse or domestic partner’s benefits and their open enrollment period has already ended? Ordinarily, you may not change elections mid-year. However, the IRS allows, and most plans permit, exceptions to this rule for certain qualifying life events, including a “change in coverage under another employer plan.” The Dec. 14 integration date is typically considered a qualifying event by other employer-sponsored plans. Please contact your spouse or domestic partner’s benefits administrator to confirm.
How do I enroll in benefits if I go out on a leave of absence before or during the initial enrollment period? You enroll Dec. 15 – Jan. 13, 2026, which is the same timeframe as your colleagues. If you don’t enroll during your initial enrollment period, you won’t have medical, dental or vision coverage, or most other elected benefits such as Critical Illness or Accident Insurance, for the remainder of the calendar year. You won’t be able to enroll in benefits until the 2027 Open Enrollment period, unless you have a qualified status change such as marriage, divorce, or birth/adoption of a child.
Is there benefits coverage for domestic partners? Yes. We offer domestic partner (same and opposite gender) coverage for most health, life and voluntary benefits.
Is there benefits coverage for an ex-spouse or grandchildren? Former spouses, a spouse from whom you are legally separated, or a spouse with respect to whom an annulment or decree of separate maintenance is in effect, and/or former domestic partners are not eligible for coverage under our plans. If any are currently covered under medical, dental, or vision, they will be sent a COBRA coverage continuation package. A grandchild of either you or your covered spouse or domestic partner must meet certain requirements to be considered an eligible dependent. Please refer to the Dependent verification documentation.
Will I need to provide dependent verification documentation for my enrolled dependents? Yes. Dependent verification documentation is required to confirm dependent eligibility for coverage. Once you submit the documentation, it takes up to three business days for a determination if you upload your documents to the Benefits Site when enrolling, seven business days if you fax it, and up to 21 business days if you mail it. You'll receive a letter at your home address explaining the process after you enroll at least one dependent in medical, dental or vision.
When do deductions for elected benefits (medical, dental, vision, etc.) start? The date you enroll in benefits affects when premiums start being deducted from your paycheck. Premiums are owed for each paycheck in the month your coverage begins, regardless of when during the enrollment period you enroll. No more than two deductions will be taken from a single paycheck.
Besides premiums, will I see any other benefits deductions on my paycheck? You might. Because you automatically receive Basic Life Insurance and AD&D coverage effective on your transition date, you are taxed on the value of the company-paid premiums above $50,000. This is called imputed income. If applicable, you will start to see imputed income deductions beginning with your second paycheck. Depending on your integration date, you may see a double deduction on your first check and a single deduction going forward.
After I enroll, when can providers verify my insurance coverage? After you enroll, network providers will typically be able to verify your insurance coverage within three business days. ID cards are not required to receive network services.
When will I receive my medical, dental and/or vision ID cards? You should receive your medical and dental member ID cards at your home address approximately two weeks after you enroll. A vision ID card will not be issued.
What if I need care before my new benefits can be verified? You will be reimbursed according to your new plan’s provisions. You’ll pay out-of-pocket (or the non-discounted amount for pharmacy) and submit your receipt(s) for reimbursement once your medical coverage is verified. If a true emergency occurs and you are taken to the nearest hospital, you will be admitted even if the hospital cannot verify your medical coverage. Even if the facility is not in the network, services will be covered at the network level until you can be relocated to a network facility. You can ask the facility to submit the claim after you’ve enrolled in benefits.
I have an upcoming procedure that is scheduled for after integration. How does integration impact that? After you enroll in a UnitedHealth Group-sponsored medical plan, you will need to notify your provider of your new medical plan and work with them to reestablish any prior authorizations. The cost share will be based on the provisions of your newly elected medical plan. If your provider is no longer in-network under your newly elected plan, you can request and submit a Transition of Care application, if eligible, to see if they will extend your coverage at network rates for a limited time.
How does integration impact access to prescriptions? Prescription coverage is through Optum Rx and most plans utilize the Standard Select network. After you enroll in a UnitedHealth Group-sponsored medical plan, you will need to reestablish any prior-authorizations. The cost share will be based on the provisions of your newly elected medical plan. Note: CVS Pharmacies are not in-network for most plans. For more details, visit Health and Pharmacy Tools.
I have a Flexible Spending Account (FSA) with Atrius. What happens to my unused balance? It will not transfer over nor will any carryover/rollover amounts. At the time of integration, you will no longer be eligible to actively participate in the FSA at Atrius. Whatever dollars you do not claim will be forfeited when the plan is terminated, as dictated by the Internal Revenue Service (IRS). To limit impact, please review your account and consider spending down your contributions, including any carryover/rollover amounts, through your last standard paycheck from Atrius – think about the medical expenses you and your covered dependents have incurred since Jan. 1, 2025, and submit any applicable receipts. You have through Dec. 31, 2025; to incur eligible expenses and the reimbursement period will be through March 15, 2026. Please contact your current FSA administrator for more information on reimbursement timing and eligible expenses.
I have an FSA with Atrius and need to spend down my contributions. What can I use the funds for?
Are Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) offered? Yes, we offer HSAs if you enroll in an HSA-eligible medical plan and both Health Care and Dependent Care FSAs.
What are Flexible Spending Accounts (FSAs): With an FSA, you can set aside pretax dollars to pay for eligible expenses. We offer the following three types of FSAs:
What’s a Health Savings Account (HSA)? If you enroll in an HSA-eligible plan, an HSA will be automatically opened for you at Optum Bank. The company contributes to you HSA on a biweekly basis (the amount varies based on whether you only cover yourself or also cover dependents) and you can also put your own money into the account by making pretax payroll contributions, up to IRS limits. You own your HSA, which means it’s yours to keep even if you switch to another medical plan, change jobs or retire.
I currently have an HSA with another administrator. Can I transfer the funds to my newly-opened HSA with Optum Bank? Yes, but you must wait until a couple of pay cycles after you enroll in an HSA-eligible plan to initiate the transfer. Optum Bank will have the information you need (including which form to use, whether there’s a transfer fee, etc.), but you should also contact your current HSA administrator to learn what steps you need to take to close out the account. Transferring funds to Optum Bank is optional – your existing account will stay with you and you can continue to use the funds to pay for eligible expenses after integration, but you’ll be responsible for the monthly administrative fee.
What will happen to my current HSA if I don’t enroll in an HSA-eligible medical plan under the new benefits package? You can continue to use your funds to pay for eligible expenses, but you won’t be able to make new contributions to the account through payroll.
I will be turning 65 soon. Do I need to enroll and start paying for Medicare? You will want to compare coverage between Medicare and your current health care coverage when you turn 65. If you defer your Social Security benefits, you will NOT automatically be enrolled in Medicare Parts A & B and can choose to defer enrollment until later. You can also wait to enroll in Medicare Part B until you leave the UnitedHealth Group enterprise without having to pay a penalty.
If I have Medicare Part A coverage, does my medical coverage through the UnitedHealth Group enterprise pay first or Medicare? If you are employed (regardless how old you are), your employer-sponsored medical coverage is primary, and Medicare Part A is secondary until you leave the company.
My spouse, who is 67, is enrolling in a Medicare Supplemental Plan and planning to enroll in Medicare Part B. Medicare needs a “L564-Request for Employment Information” form, so my spouse can avoid a penalty and enroll now. Who do I contact? After integration and once you have enrolled in a medical plan, you can call the respective call center and request they complete a CMS_L564 Form for your spouse. The representative will complete the form and mail the original to your home address in approximately 10 business days. If you need the form before integration, please contact your current benefits administrator.
