Patient Advocate Nurse conducts telephonic clinical review as appropriate to evaluate for delays in care, facilitate discharge planning, and arrange for alternate services, assess the quality of care and services and identify admission and readmission risk.
Risk Manager Nurse
Identify individuals with unmet health care needs that are at-risk for hospital admission/readmission and assess the quality and appropriateness of post-discharge care and services.
RN Inpatient Care Advocate
Provides telephonic care coordination.
